  • You may qualify if:

    • You're on Medical Assistance (MA)

    • You have a mental health condition, disability, or substance use history

    • You struggle with finding, keeping, or maintaining housing

  • No — you do not need a waiver or active case manager.
    We can work directly with:

    • Clients referred by family, shelters, or treatment programs

    • Clients who don’t receive waiver services (like CADI or BI)

  • To start Housing Transition or Sustaining, we need:

    • Professional Statement of Need (PSN) – signed by a qualified provider

    • Housing-Focused Person-Centered Plan (HFPCP) – from a Housing Consultant

    • Proof of active MA–PMI number or insurance card screenshot

    If you don’t have these, we can help you get them.

  • We accept referrals from:

    • Case managers

    • Social workers

    • Treatment centers

    • Shelters

    • Family members

    • Clients themselves
